There are 2 easy ways to schedule your 1st appointment:

Online Self-Scheduling

2. Call (919) 869-7300 to leave a message for scheduling assistance;  call backs are typically within 1-2 business days

​

By electing to be out-of-network for all insurance companies, we are better able to serve our patients with extended visits (60-90 min) that reduce the need for multiple follow-up appointments and provide advanced therapies not typically covered. 

​

All forms of payment are accepted including FSA & HSA cards. Payment is due the day of service. All invoices with codes are available in the Patient Portal to self-submit for out-of-network coverage. 

​

Vitality PT is a non-participating Medicare Provider. 

• Choose "INITIAL PT EVAL & TREAT" when scheduling first appointment online

​

• Make sure you signed up for the correct location (Chapel Hill or Raleigh office)

​

• Please fill out ALL paperwork online through the Patient Portal prior to your appointment 

​

• Plan on spending 1.5 hours that day

​

• YES! Bring the baby :)
